Chief Executive Officer
Dr. Buhari Mohammed’s dedication to providing everyone access to quality healthcare, regardless of ability to pay, began as a young primary care physician. His passion to increase patient access and experience lead him to a career in healthcare administration where he has spent over 20 years enhancing the healthcare needs of the community. A results-oriented and data-driven decision maker with a proven track record of success, he is not only a change agent, but a strategic thinker that embodies service integration and the Federally Qualified Health Centers’ mission. Dr. Mohammed became CEO of Heart of Ohio Family Health in 2018 and has helped HOFH lower the cost of care, improve clinical quality, and launch the opening of two additional health centers.
Dr. Mohammed obtained his MD from Ege University, MBA from Capital University, and completed a Community Health Center Executive Fellowship from the University of Kansas Medical Center. He is a member of the YMCA of Whitehall Board, Franklin County Juvenile and Family Recovery Court Advisory Board, United Way of Central Ohio Fitness and Nutrition Board, The Ohio State University Medical Dietetics Advisory Board, the American Diabetes Association of Central Ohio Advisory Committee, and the Mid-Ohio Food Bank Advisory Board.
Chief Operating Officer
Yammah Morgan has been passionate about making a difference in the personal and professional lives of others, especially if it improves the community. Yammah has over 14 years of management experience specializing in customer service, strategic planning and project management. As COO, Yammah directs, supervises and coordinates the overall clinical and business operations for Heart of Ohio Family Health. Prior to joining HOFH, Yammah managed the compliance program of a large healthcare company where she traveled across the country training other centers on audit readiness.
Yammah holds a bachelor’s degree in Economics from Wright State University, an MBA from Mississippi College, and a Graduate Certificate in Health Services Management. Yammah held the position of Vice President with the National Black MBA Association. She has also volunteered and played vital roles in the Immune Deficiency Foundation and National Hemophilia Foundation.
Dr. William L. Washington, MD
Chief Medical Officer
Dr. William Washington has a strong commitment to improving the health status of individuals and families who are underserved. “I believe we must always put our patients’ needs ahead of our own. Every patient, no matter their circumstance in life, deserves to be listened to and cared for with dignity and respect. I was drawn to Heart of Ohio Family Health because my personal beliefs align with their mission of high-quality, holistic and sensitive care which is operationalized through their vision statements.”
Dr. Washington has served as Chief Medical Officer for HOFH since 2015. Prior to his career at HOFH, Dr. Washington practiced as a physician in various capacities. For over a decade, he owned and operated his own private practices in Columbus, Ohio, where he served a large number of minority, Medicaid, low income, and non-English speaking patients.
Dr. Washington is a member of the American Association of Family Practitioners and The National Medical Association.
Director of Clinical Services and Quality
David is passionate about creating a healthcare team that cares for the whole patient by addressing social determinants of health, healthy lifestyle, and behavioral health in addition to best practice medical care. He has over eight years of experience in healthcare administration and clinical practice as a registered dietitian in the FQHC setting. He graduated from Indiana University with two Master’s degrees in Business Administration and Strategic Management. He has a Bachelor’s in Health and Rehabilitation Sciences and majored in Medical Dietetics. Additionally, he has the following licenses and certifications: Registered Dietitian, Licensed Dietitian, Lean Six Sigma Blackbelt in Healthcare, Certified Professional in Healthcare Quality, and Board Certified Specialist in Obesity and Weight Management.
He currently manages all the ancillary clinical services and quality improvement initiatives at Heart of Ohio Family Health Centers.
Director of Marketing and PR
Terry Anderson joins Heart of Ohio Family Health with extensive professional and community-based experiences as a foundation for culturally competent vision, proactive and results-oriented strategic planning, and passionate leadership in areas that align with the Heart of Ohio Family Health mission. He also offers an extensive communication and multimedia background that has been utilized to expand the reach of organizations through print, media, and web-based initiatives.
He was employed with Columbus City Schools’ Communications and Engagement Office for over a decade, and has over 30 years of community impact programming, educational support, and various communication experiences that range from designing and producing educational publications for a national educational publisher to devising outreach approaches to enhance the art and cultural prosperity of central Ohio with the Greater Columbus Arts Council…
While serving as the Director of Communications and Marketing with Ohio United Way, he provided oversight and strategies —specifically in the area of health and human service concerns— that supported and informed Ohioans about the innovations and trends in the health care enterprise. He was also responsible for researching, developing, designing, and distributing various publications that targeted specific audiences throughout the state.
With these varied experiences, he is eager to embrace and enhance the vision and direction of the Heart of Ohio Family Health leadership, administration, staff, and Board.
Mr. Anderson resides in Whitehall, and serves on the City of Whitehall Planning Commission, Whitehall Education Foundation Board, Ohio Alliance for Arts Education Board, and has volunteered as a board member or in a governance capacity for several local, state, and national non-profits. over the past 30 years.
Jaclyn H. Woodard
Senior Director of Human Resources
Jaclyn H. Woodard has more than 30 years of leadership experience in the healthcare industry and a passion for mentoring, sharing, and engaging with staff. Jaclyn began her career as a Case Manager at a community mental health center before becoming a Medical Practice Manager in both private practice and hospital. Her extensive health experience includes Benefits Pharmacy Manager Administrator and VP of Marketing and Chief Operating Officer (COO) for a virtual pharmacy with contracts in many states. She has been leading Human Resources for HOFH since 2012.
Jaclyn has a Bachelor’s Degree in Social Work, a Licensed Social Worker (LSW), and holds a Master’s Degree in Healthcare Administration. She is a member of the Society for Human Resources Management, and holds a Certificate in Internal Investigation. Other Civic and Social organizations. Jaclyn is a member of The Links Incorporated, Columbus Chapter, where she has served a Vice President, Financial Secretary, Social Chair, National Trends and Services Chair etc. She is a member of The National Smart Set, Columbus Chapter, serving two terms as Chapter President, Secretary, Social Chair etc. and in 2019 served as Chair of National Conclave in Columbus, Ohio.
Chief Finance Officer
Angelita (Lita) Thomas has 31 years of experience in the accounting and finance field, with 20 of those years in fund and non-profit accounting as a Finance Manager, Interim Finance Director and Interim CFO. As Director of Finance for Heart of Ohio Family Health, she is responsible for all accounting operations and ensuring that reported results meet HRSA reporting requirements and comply with generally accepted accounting principles.
Lita is a graduate of Ohio Dominican University where she earned a B.S. in Business Administration and is currently working toward her Certified Management Accountant designation.
Senior Director of Strategic Initiatives
Dr. Whitted’s career began as a molecular biologist and she carries that training and experience of critical thinking and curiosity to her work in health care. She spent over 30 years at Planned Parenthood affiliates in Indiana, Texas, and Ohio in many roles including Director of Education, Director of Pubic Affairs, and as CFO. She worked for 5 years at PrimaryOne, a Central Ohio Federally Qualified Health Center, where she managed clinics in underserved neighborhoods. She and her team implemented behavioral health integration into the clinics utilizing various models and levels of integration. Through these integration projects, she collaborated with numerous behavioral health organizations in the Central Ohio area. She also successfully implemented Comprehensive Primary Care, an Ohio payment reform pilot. She provided consulting services in the area of healthcare for three years until joining Heart of Ohio Family Health as Senior Director of Strategic Initiatives.
She received her BS in Microbiology from Purdue University. While there, became a member of Phi Beta Kappa. She received her MA in Plant Physiology from Indiana University, researching algal genetics, her MBA from Notre Dame and her Doctorate in Public Health from the University of Texas. While encouraged to stay in academia to pursue research, she is passionate about incorporating evidence-based practices established through research into everyday medical care.
Grant Seredick is the Risk/Compliance Manager who oversees compliance, risk management, patient relations, and provider credentialing/privileging. Grant believes that quality healthcare should be accessible no matter what barriers Heart of Ohio Family Health’s patients face. Grant holds a B.A. in Public Health Education and a Master’s Degree in Healthcare Administration from Otterbein University. Prior to his career at Heart of Ohio Family Health, he managed several government funded grants as a Health Educator at a local health department. Additionally, he currently serves on the board of the Society for Public Health Education’s Ohio chapter.
Dalia Elkhairi, MD
Director of Women’s Health
A Cleveland, Ohio native, Dr. Elkhairi received her medical degree from The Ohio State University in 1997. She completed her Obstetrics and Gynecology residency at The Ohio State University Wexner Medical Center in 2001 and then was in practice at University Hospitals of Cleveland through 2004. After a move to Lexington, Kentucky, she was an Assistant Professor of OB/GYN at the University of Kentucky Medical Center. She was instrumental in developing a minimally invasive Gynecology surgery program at the University of Kentucky as well as maintaining a thriving practice with the Lexington Clinic. After returning to central Ohio in 2010, she practiced at Licking Memorial Hospital and with Womankind OB/GYN. She has been with Heart of Ohio Family Health Center since March 2016 serving as the Women’s Health Director. She has courtesy privileges at Mount Carmel East and Grant Hospital.
Dr. Elkhairi specializes in complete women’s care from teen care, contraceptive management, prenatal care to menopausal and post menopausal issues. She is board certified by the American Board of Obstetrics and Gynecology and is a Fellow of the American College of Obstetrics and Gynecology. She lives in New Albany with her husband, Ray and their two children and three dogs.
