Heart of Ohio Family Health delivers comprehensive, quality medical care to patients of all ages, from newborn infants to older adults. If you do not have a regular doctor, we encourage you to begin visiting our team for regular health screenings. Wellness checkups allow us to find diseases early when they are often easier to treat. We’ll be there for you and your family for urgent health issues as well as everyday health education and disease prevention. As proponents of patient-centered care, we account for each individual’s preferences, goals, and values during the care planning process.

We Provide A Sliding Fee Scale And Other Pathways Of Access To Quality Care

At Heart of Ohio Family Health, we accept Medicaid and provide government-subsidized care on a sliding fee scale (click here) for qualified patients. Our certified application counselors help patients to determine their eligibility for Medicaid, Medicare, and other health insurance programs. Patients can easily contact our doctors and access records and test results with our secure electronic portal. Simply call (614) 235-5555 to schedule your appointment for a regular checkup, or health concern, at one of our six locations in central Ohio.

Personalized Medication Management
Our clinical pharmacist can work with you and your healthcare provider to develop a personalized medication plan and lab testing to help meet health goals and avoid unwanted side effects. Our pharmacist can assist in managing medications for a variety of health conditions, including:

Additionally, our pharmacist can provide a comprehensive medication review and education on medications.
