Certified Application Counselors (CAC) are part of Heart of Ohio Family Health (HOFH) staff designated to obtain financial information and provide assistance to all patients.

Our Certified Application Counselors (CAC) offer financial assistance to all Heart of Ohio Family Health patients. They can enroll you and your family in Marketplace insurance plans, and Medicaid. If you do not qualify for health insurance, they can sign you up for our sliding fee program that provides discounted costs for healthcare services depending on your income.​

Community Health Workers (CHW)

One's health is impacted by more than just the medical care that they receive. Our Community Health Workers assist our patients by addressing all of the social drivers of health. If you have difficulty with transportation, food, clothing, housing, financial difficulties, cribs, or baby items, our community health workers can connect you with resources in the community.

Mid-Ohio Farmacy

Heart of Ohio Family Health partners with Mid-Ohio Farmacy (a free program that helps patients to receive fresh fruits and vegetables every week to enhance their health).

What are the benefits of joining Mid-Ohio Farmacy?
Fruit and vegetables are nutritious, but can be expensive! Getting health foods every week from the Mid-Ohio Farmacy program will help you get the food you need to stay healthy and help control your blood pressure, cholesterol, diabetes, and weight.
What's the difference between this program and visiting food pantries?
The Mid-Ohio Farmacy program is different than normal food pantry visits in the following ways:

How do I join?
Schedule a visit with one of our health providers to secure a Mid-Ohio Farmacy card to scan at the various food pantries in Central Ohio.
How do I receive my produce?
We will provide a list of participating Mid-Ohio Farmacy locations and their operating hours that will assist with accessing fresh produce for your household.
