Heart of Ohio Family Health has five health care clinics to serve the needs of new and current patients. We provide services in primary care, pediatrics, behavioral health, substance use disorder, OBGYN, diagnostic laboratory, pharmacy, dietetics & nutrition, and interpretation.

Do I have to pay for my Visit?We have a sliding fee scale for those who qualify. Sliding scale fees are based on proof of household income relative to the Federal Poverty Level and family size. Co-payments are collected at the time of service, with insurance co-pays as shown on your insurance card or a $25 minimum co-payment for those without health insurance. All insurances are accepted. Are there interpreters on Site?Yes, we have on site interpreters in Nepali, Somali and Spanish at our locations. We also have a phone interpret system for all other languages. For further questions on interpretation services, please call (614) 235-5555.

Is there any health insurance assistance?Medical Health Care Open Enrollment is the first two weeks of December. We have trained Certified Application Counselors who are able to help patients look for health coverage options. These services are free and open to anyone who needs assistance. To schedule an appointment with a Certified Application Counselor, visit your nearest Heart of Ohio Family Health location or call (614) 235-5555.

Where can I access my past medical records?You can access your medical records through our patient portal, ​FollowMyHealth​. You will need to make an account to access all medical information. For further questions on requesting medical records, please call (614) 235-5555.
