Family Practice
We provide the health care your family needs every day(delete comma here) to keep you healthy for school, work, play, or an active retirement. We want to be your “medical home,” the place you think of when a health care need arises. We also want to help make the community a healthier place to live so we participate in health fairs, health education and other free events to keep you informed about healthy ways to live.
We’re here for the minor illnesses and injuries of life, and we also have special support programs for people living with diabetes and other serious, life-changing conditions.

Information for Health Professionals
At Heart of Ohio Family Health Centers, essentially all primary care services are provided on-site, including: primary medical care; diagnostic laboratory; obstetrical/gynecological care (obstetrics at the Whitehall location only); screening; case management; transportation; translation; and other enabling services.
Referral systems are in place for other laboratory, tuberculosis, dentistry, mental health crisis and substance abuse, and other specialty services. We collaborate with a community-based mental health provider to provide mental health trauma care services for refugees at our Capital Park facility. We have special expertise in cross-cultural diabetes management, including intensive case management and native-language educational materials for our patients who speak Spanish and Somali.
We have an ethnically and racially diverse staff with cultural diversity training. Our cultural competencies include bilingual staff onsite every day for our Spanish and Somali patients, and the ability to quickly access medical interpretation for patients speaking 30 additional languages.
We gladly accept most private insurances and self-pay patients. We have a sliding fee scale based on income relative to the Federal Poverty Level.
