About Heart of Ohio Family Health Centers

Heart of Ohio Family Health Centers™ currently operates two sites: Capital Park Family Health Center and Whitehall Family Health Center. Capital Park Family Health Center was established in July 2003 in collaboration with Mount Carmel's Community Outreach Program. The center was initially funded by Mount Carmel with limited services but quickly expanded to full-time operations with the award of Federally Qualified Health Center status and Consolidated Health Centers funding from the Health Resources and Services Administration in 2003.

As of Nov. 1, 2007, Capital Park Family Health Center became an independent Ohio non-profit organization and no longer part of Mount Carmel Health System. Due to our need to expand and operate additional sites, we adopted the corporate name Heart of Ohio Family Health Centers™.

Both health centers currently provide primary medical care; diagnostic laboratory; referral for dental services; screening and maintenance for mental health issues; case management, transportation, interpretation and other enabling services. In addition, the Whitehall Family Health Center provides OB/GYN services and limited dental services.

In Feb. 2009, Heart of Ohio Family Health Centers™ opened its second federally qualified health center in Whitehall. The Whitehall Family Health Center provides primary care as well as OB/GYN services.

Federally qualified health centers are special, nonprofit, private entities with federal certifications and support that act as primary care centers. They are self-sustaining and rely on community support for additional funding.
